What Is CARE Patient Information System

CARE Patient Information System is a digital hospital system that helps healthcare professionals — especially doctors and nurses — manage the prescription, administration, and monitoring of medications for inpatients within a ward.

It ensures that each patient receives the correct drug, at the correct dose, at the correct time, while tracking inventory and minimizing medication errors. It also supports coordination between the ward, pharmacy, and clinical teams.

How Does It Work

This system operates by coordinating doctor prescriptions, pharmacy dispensation, and nursing administration, all within a unified workflow.

1. Medication Orders by Doctors

  • Doctors prescribe drugs directly into the system during ward rounds or patient reviews.
  • Prescriptions include:
    • Drug name and dosage
    • Route of administration (oral, IV, injection, etc.)
    • Frequency and duration
  • System checks for:
    • Drug interactions
    • Allergies
    • Duplications

System Integration: Medication orders are sent in real time to the pharmacy system.

2. Pharmacy Processing and Drug Dispensation

  • The hospital pharmacy receives the digital order.
  • Verifies and dispenses the drugs to the ward, either:
    • Per patient (unit dose system), or
    • Bulk supply to the ward stock

Inventory Tracking: The system tracks medicine usage and stock levels in each ward.

3. Medication Administration by Nurses

  • Nurses receive a medication administration schedule for each patient.
  • Prescriptions are sent electronically to pharmacies (e-prescribing).
  • The system checks for drug interactions, allergies, and proper dosing.
  • At scheduled times, nurses:
    • Verify patient ID
    • Confirm correct drug, dose, and route
    • Administer the medicine
    • Record the administration in the system (MAR – Medication Administration Record)

Barcoding (if available): Barcode scanning of patient wristband + drug package helps prevent errors.

4. Monitoring and Documentation

  • Nurses document any:
    • Side effects
    • Missed doses
    • Patient refusals
  • Nurses document Room and bed allocation
  • Doctors can monitor medication response and adjust treatment accordingly.

5. Medication Adjustments or Discontinuation

  • Doctors can:
    • Modify the drug, dosage, or frequency
    • Discontinue a medication
  • All changes are logged, timestamped, and updated in the nursing and pharmacy modules.

6. Ward & Bed Management

  • Monitor bed occupancy and patient transfers.
  • Manage ward admissions, discharges, and bed assignments.

7. Operation Theater (OT) Scheduling

  • Schedule and manage surgeries.
  • Track pre-op and post-op details, checklists, and staff assignments.

8. Nursing Station

  • Nursing care planning, medication schedules, vitals monitoring, and shift handovers.

9. Test Orders and Results

  • The provider can electronically order lab tests or imaging.
  • When results are ready, they are automatically uploaded into the patient’s record.

10. Drug Stock Management

  • Tracks inventory of commonly used drugs stored in the ward.
  • Issues alerts for:
    • Low stock
    • Expiring medications
    • Reordering thresholds

Handled by: Ward nurses or pharmacy aides.

11. Reporting and Audit Trails

  • Generates reports on:
    • Medication usage per patient/ward
    • Administration compliance
    • Medication errors or delays
  • Useful for audits, quality assurance, and improving prescribing patterns.

12. Clinical Documentation

  • During the visit, the healthcare provider uses the system to:
    • Record symptoms, diagnoses, and vital signs
    • Add notes and treatment plans
    • Prescribe medications
    • Order lab tests or imaging

🔹 Doctors

  • Prescribe medications
  • Monitor effectiveness
  • Adjust treatment as needed
  • Discontinue or change drugs based on progress

🔹 Nurses

  • Administer medications on schedule
  • Monitor and document patient reactions
  • Monitor bed occupancy and patient transfers.
  • Manage ward admissions, discharges, and bed assignments.
  • Ensure the 5 Rights: right patient, right drug, right dose, right route, right time
  • Maintain drug stock levels in the ward

CARE ensures that:

  • Doctors prescribe safely and efficiently
  • Nurses administer medications accurately and on time
  • Pharmacy supplies are managed systematically
